SUMMARYThe Case Manager provides group therapy and related
documentation services, along with additional ancillary services.
The Case Manager is a highly qualified individual with experience
regarding the nature of alcoholism and drug addiction. The Case
Manager will need to be familiar with 12-Step Recovery programs and
have knowledge of documenting group notes in an EMR system. Must
have the ability to work as a team member and work collaboratively
with the Primary Counselor and Clinical Director in providing
quality treatment services and client care.
ESSENTIAL DUTIES AND RESPONSIBILITIES1. Coordinate and provide care
that is safe, timely, effective, efficient, equitable, and
patient-centered2. Handle case assignments, draft service plans,
review case progress and determine case closure3. Communicate
patient needs to a variety of care team members and follow up
accordingly.4. Manage discharge plans upon completion of
treatment.5. Attends clinical and administrative meetings for
inter-departmental sharing of information.6. Work collaboratively
with patients, families, therapist, and referral sources to ensure
high quality care7. Facilitate multiple care aspects (case
coordination, information sharing, etc.)8. Maintain the patient's
comprehensive clinical record through detailed documentation9. Help
patients make informed decisions by acting as their advocate
regarding their clinical status andtreatment options10. Promote
quality and cost-effective interventions and outcomes11. Record
cases information, complete accurately all necessary forms and
produce statistical reports12. Helps navigate legal issues that
patients are currently facing up to an including appearance in
courthearings.13. Adhere to professional standards as outlined by
protocols, rules, and regulations14. Act as the patient's advocate
as it relates to insurance coverage and financial assistance15.
Continually monitor and evaluate each client's progression through
their case management plan, anddevelop corrective action revisions
to the plan as needed16. Arrange abeyance with social services,
health, and governmental agencies17. Performs other duties as
assigned by the supervisor, which are consistent with the position
and incompliance with agency policies and procedures.
EDUCATION / EXPERIENCE* CAP or CAC, preferred* Bachelor's Degree in
a human services field, preferred* Minimum 1 year of full-time
employment working in human services or in behavioral health
sciences* Knowledgeable of Department of Children and Families
(DCF) guidelines and Joint Commission standards* Must be familiar
with 12 Step Recovery programs* Knowledge of Kipu - preferred

Salary Description
$40,000 - $55,000 yearly
